ITEMS REQUIRED FOR HOME INSPECTION
Current certificate of registration.

CRC letter(s) Criminal Record Check for yourself, other adults in the home over the age of 18, staff, substitutes, and/or others visiting in the home on a regular basis.

Current Georgia driver's license if providing transportation.

DHR enrollment forms on all children enrolled (including full-time, part-time, after school children, and/or drop ins) Information to include the following: Child's name, birth date, parents name, home and business addresses, telephone numbers: Current age-appropriate immunizations, Emergency medical treatment authorization, Documentation of allergies and other known medical problems, Transportation aggreement, and Release names.

Notarized non-pay statements for any children attending at no pay.
Related care statements for any related children attending at no pay.

Infant information forms for any child under 12 months of age.

Medical authorization for any children receiving medicine during your home daycare hours.

Pet vaccination papers.

Documentation of well and septic tank approval if applicable.

Training certificates for annual 10 hours of required training.

CPR (Infant/Child/Adult) and First Aid Certificates.

Emergency Plans including fire, severe weather, loss of electrical power and loss of water, death of a child, serious injury or accident of a child, loss of a child.

Posted phone numbers including ambulance or rescue squad service, physician or hospital, local fire department, local police department, county health department, regional poison control center (911 may be in lieu of the phone number in your areas of stated services that have a 911 emergency number)

No smoking sign.

Documentation of fire drills.

Documentation that staff/substitute have received orientation of the family daycare rules, policies regarding discipline, injuries, illnesses, and release of children, written plans for handling emergencies, age-appropriate information about any child's specific health needs.

Training certificate in lifeguarding for provider or adult supervising the water activities when swimming and/or water activities over 2 feet deep are provided to the children.

First Aid kit containing: scissors, tweezers, gauze pads, thermometer, adhesive tape, syrup of ipecac (with current date), assorted Band-aids, insect sting preparation, antiseptic cleaning solution, antibacterial ointment, triangular bandage to make a sling, disposable rubber gloves, cold pack, First Aid manual.

Smoke detector.

Fire extinguisher UL Rated (3-A: 40-B:C)

Documentation of an accident or serious illness that occurred while a child was present in the family daycare home that required professional medical attention.
